Obtaining Permission to Skate Form for AAA
Submitted By Joanne Rey-Reiter on Sat 04 16, 22
You must register with Aurora for the 2022-2023 season before a PTS form will be sent to you. If the player is successful in making a AAA team, they must notify the Aurora MHA Office and a refund will be issued.

IF YOU HAVE PLAYED ON A “AAA” TEAM FOR THE LAST TWO OR MORE CONSECUTIVE YEARS, please register and choose the Cheque or Cash payment option (no payment required). Please advise the office if you make a AAA team again and where as soon as possible after the tryouts.

If you require a PTS form, please email players name, date of birth, home  address and  the team last played for to the office at [email protected] or [email protected]

The Ontario Minor Hockey Association (OMHA) requires that players trying out for a AAA team obtain a Permission to Skate (PTS) form prior to stepping on the ice. In order to receive a PTS form from Aurora MHA, the player must be a resident of Aurora.

For players U10-U13, the Aurora MHA can provide a PTS form for the York Simcoe Express. If a player is released from YSE tryouts, they can request an OHF Waiver form from YSE to allow them to tryout with an adjoining AAA Zone.

For players U14-U16 -they may tryout with any OMHA AAA team although a PTS form is still required.

For players at U18 to attend AAA tryouts, a PTS form will be issued in accordance with OHF regulations K1-K27. Player movements to another OHF Member must follow current OHF Waiver regulations.

For Junior PTS, if a player played on a AAA team this past season, the request for the Junior PTS must be done through the AAA Association. If the player played for their home centre, then we issue the PTS form.

If you require a PTS form, please email players name, date of birth, team last played for to the office at [email protected] or [email protected]
